Who is BestCare Central Coast Home Health?
BestCare Central Coast Home Health is a community based not-for-profit organization that provides intermittent skilled nursing care and rehabilitation services to residents of San Luis Obispo and Northern Santa Barbara Counties.
BestCare Central Coast Home Health is a service of Wilshire Health & Community Services, Inc. BestCare is state licensed, certified for Medicare and Medi-Cal, and is accredited by the Community Health Accreditation Program (CHAP).
What is Home Health Care?
Home Health Care comprises a variety of services that include skilled nursing care, physical, occupational and speech therapy, as well as assistance with personal hygiene care performed by certified home health aides. Home Health patients enjoy the privacy and comfort of their own homes, while professionals provide them with personalized care, treatment and instruction.
What are the goals of home health?
BestCare professionals teach the patient and caregivers how to manage their medical regime. BestCare works closely with the patient in fostering independence and promoting self-care.
BestCare’s goal is to prevent escalation of a condition in order to avoid hospitalizations. BestCare clinicians work with patients and their physicians to ensure continued progress is made.
Who is eligible?
A patient can receive services through their insurance, Medicare, or Medi-Cal coverage when they meet the following criteria:
• The service must be prescribed by the patient’s physician.
• The patient must be in need of skilled nursing care or rehabilitative therapy care.
• The patient must be home bound (difficult to leave home and is a taxing effort).

• Self - pay
*Under Medicare and Workers’s Compensation benefits, there is no co-payment required.

When the time comes for Home Health Care, your hospital must provide you with a choice of home health agencies — even though the hospital may have their own home health agency and the best choice is BestCare.
