Download an application or contact our Human Resources Department at 805-782-8600, ext.122 to request an application. Once you have received the application, please complete and return it to the Human Resources Department at your earliest convenience. Please include any reference information you have available that will assist us in the selection and qualification process. If you are applying for HHA or a Nursing position, also provide us with a current copy of your CNA/HHA certificate or nursing license.
